Registration Form

Upon proper completion of this form and acknowledgement of all policies therein, you will be authorized to submit a deposit to claim your workshop spot. No payment may be submitted without completing this form and there will be no exceptions to any of the policies outlined below.

The energies in this workshop can affect people in different ways. It is a condition of booking that students attending this workshop inform the facilitator if they have any past or current history of the following: food or other allergies, conditions such as diabetes, epilepsy, mental illness, other chronic conditions that require medication or relevant medical or personal information that the facilitator should be made aware of. This is both to ensure that proper arrangements are made in the event that this is necessary, as well as for liability purposes. Use the space below to provide this information (if you have nothing to advise the facilitator, put "N/A").

By checking "I agree", I acknowledge that I have taken full responsibility to communicate any additional needs, conditions, or relevant information to the workshop facilitator in the space provided above. By checking "I agree", I am also assuming responsibility for any additional health and liability factors involved in my workshop attendance such as travel, illness, injury, etc. in my travels to, participation in, and travels from the workshop space. If I check "I do not agree" I accept and acknowledge that I will not be allowed to participate in the workshop.

In order to officially register for the workshop, a deposit of $150 of the tuition cost is due upon completion of this form. I understand that if I do not complete payment in full 2 weeks prior to the workshop date, that my spot will be forfeit and opened up to someone on the wait list with no guarantee that the spot will remain available should I seek to maintain my spot. I also understand that if I should drop out at any point after registration, any payment I have put forward will not be rolled over to a future workshop. By checking "I agree", I acknowledge the aforementioned payment schedule policy. If I check "I do not agree" I accept and acknowledge that I will not be allowed to participate in the workshop.

By checking "I agree", I acknowledge that I am agreeing to pay the full workshop amount before the workshop date and that no refunds will be given unless the facilitator has to cancel the workshop. I also acknowledge and accept that if unforeseen events effect my ability to make my final payment prior to the workshop date and I attend that I will incur a $50 late fee for each week it is not paid, that my manual and certificate will be held by the facilitator until the remainder is paid, and that I am responsible for paying the cost to ship the held materials. I also acknowledge that any monies put forward as deposit and any payments made leading up to the workshop are forfeit in the event that I do not attend. If I check "I do not agree" I accept and acknowledge that I will not be allowed to participate in the workshop.

By checking "I agree", I acknowledge and accept that any verbiage in the materials taught or any spiritual, political, or other sensitive opinions expressed by other workshop attendees during the course of the workshop are not a reflection upon, nor the responsibility of Hoaiuyen Nguyen or any part of her business entity, Meraki Soul Centered Healing, or any affiliated projects. I therefore accept full responsibility for anything I choose to express, discuss, or debate with other workshop attendees and the potential consequences of those expressions. If I check "I do not agree" I accept and acknowledge that I will not be allowed to participate in the workshop.*
